1. SESSION TITLE: (Provide a short, concise title, no more than eight words). Note: Titles may be edited during development process.

Provide a detailed overview (no more than 300 words) of the program (beyond learning objectives you listed) and discuss how the subject matter will be delivered utilizing the presentation format/approach you selected above. 14. Speaker Terms and Conditions - PLEASE READ: Proposal submitters are asked to keep the meeting dates available until notified of the status of their proposals until November 2018. If selected to participate in the program, speakers agree to: • Incur all expenses related to the development and implementation of their sessions; LeadingAge California cannot pay honorarium, travel, per diem or other costs for speakers. • Accept ‘Daily’ comp registration, or $200 discount for ‘Full’ registration. Speakers are entitled to a complimentary daily registration on the day of their session or a $200 discount toward a full registration category. The complimentary daily registration and discount toward a full registration category is limited to one (1) presenter for a one-hour session, and two (2) presenters for a two-hour session. All additional speakers must be approved by LeadingAge California. Additional speakers will have to pay the full registration rate. • That my draft outline of my presentation is due to LeadingAge California by February 20, 2019. • Forward session materials to LeadingAge California two weeks prior to the conference for attendees to access/download. • Accept that you must have written permission from the owner to use copyrighted materials (“ “) in presentations or handout materials. I understand it is prohibited by laws and will provide LeadingAge California with this permission in writing. Failing to do so could make my presentation and participation ineligible. Additionally, I would pay related fines or cots by including copyrighted materials that weren’t approved by LeadingAge California as stipulated. • Inform LeadingAge California within 60 days prior to the presentation if, for any reason, the session will not be presented and will find a suitable replacement. • That LeadingAge California may take photographs of my session for publicity and promotional purposes. I agree to have my image used to promote or represent LeadingAge California sponsored events, in publications and on the LeadingAge California website. • Accept that only basic audiovisual will be provided: LCD projector, screen and microphone. LeadingAge California does NOT provide laptop computers. Wifi may not be available. • Understand the session may be recorded (video/audio) by LeadingAge California for educational purposes. I give my permission for my session to be recorded in whatever way LeadingAge California so chooses. • That promoting a company, service or product during the session is prohibited. It both detracts from a positive learning experience for the attendees and risks putting you and your company in a less than positive light. • Introducing myself and my session, and make any conference related announcements asked of me by LeadingAge California staff. LeadingAge California will scan for CEUs at the start and conclusion of each session.
